Sometimes, you have approach things from a different angle. Instead of being reactive, think of how to be proactive. Limiting the entry points for failure means looking for ways to cut down on mistakes, or at least react quickly to mistakes. It means looking for potential flaws before they become headaches. It means thinking like a hacker.
